Submit a Claim
The deadline to submit a claim was August 21, 2025. Claims submitted after the deadline will be deemed untimely and may not be accepted.
If you submitted a timely Claim online, you should have received an email with a confirmation code for your completed submission. Be sure to keep your confirmation email and code for future reference if you have any questions about your Claim Form.
All valid claims were paid on December 17, 2025 via check or digital payment. If you submitted a claim and have not received your payment, please contact us.
Remember: All Claim Forms must be submitted online or postmarked no later than August 21, 2025.
